Steps to Becoming a MAC Member

For Morgan’s MAC to provide a tailored and personalized care program, we have implemented a simple process to get MAC members started. Whether you are signing up for yourself or on behalf of an individual with disabilities, we have made the sign-up process as seamless as our services.

Our Eligibility Process

To become a MAC Member, all interested individuals must first complete a Pre-Screen Form. This form can be submitted in one of three ways: by calling Morgan’s MAC directly at 210-817-3935, visiting our facility in person, or filling out the Interest Form on our website. Once the Pre-Screen Form is submitted, an Intake Navigator will contact you within 10 business days to review your information and conduct a full intake assessment for enrollment.

Get Assigned a Navigator

After an applicant has been approved for enrollment, the Intake Navigator will assign a dedicated MAC Navigator to begin care coordination. The assigned Navigator will reach out to the new MAC Member to complete a comprehensive MAC Member Assessment, identifying key areas where support is needed. Using our Morgan’s MAC Care Model™, the Navigator will then develop a personalized MAC Care Plan, which includes referrals to MAC Partners and community organizations. This ensures a seamless, closed-loop referral process.

The MAC Care Plan

The MAC Care Plan is a personalized roadmap that identifies and outlines the referrals a MAC Member needs, based on their assessment. These referrals connect members to both MAC Partners and community organizations who provide essential services and support. All referrals are actively tracked and managed through The MACNav™ System—our proprietary software designed to facilitate seamless collaboration among partners. This system ensures closed-loop referrals and promotes successful outcomes for every MAC Member.
